Finegreen are proud to be partnering with a leading independent health and social care provider who are seeking to recruit an Intermediate Care Support Manager for the North West region.

The Intermediate Care Support Manager is responsible for leading and supervising the excellent standards of care to patients and their families, ensuring the effective provision of nursing care for patients requiring rehabilitation. The IC Support Manager is supervised by an IC Clinical Operations Manager and overseen by the leadership of the home and the operational leaders of the service.   

The IC Support Manager will act up for the IC Clinical Operations Manager regularly taking charge of a nursing caseload when the relevant competencies have been achieved. The role carries continuing responsibility for the comprehensive assessment of care needs, the development, implementation and evaluation of care programmes. The IC Support Manager is accountable for standards of care and for determining the activity of the support staff, in accordance with individual rehabilitation plans, company policy, NMC guidelines whilst promoting privacy and dignity. 

 

Key Responsibilities:

 

  • To lead and motivate a team to deliver high quality and skilled nursing/rehabilitation care and support to patients and their families/ carers for defined patients;  
  • Liaise with community teams, GP surgeries, statutory and voluntary agencies in the provision of care to patients and their families/carers in order to facilitate a safe discharge;
  • To undertake holistic assessments of the patient, using appropriate documentation according to my clients Health Care’s Quality of Life Programme and other policies and procedures. This may include: falls, continence, wound care and outcome measure assessments; 
  • Fully responsible and accountable for the teaching of, and the giving of information to patients and their families, so they may be able to make informed decisions for their care;
  • Develop, implement and evaluate and progress plans of care/treatment or one-off interventions for patients. 
  • Ensure the care/treatment plans are formulated with the agreement of the patient in order to ensure concordance to facilitate patient choice and self-care where possible;  
  • Oversee all aspects of medicine management, ensuring medicines are appropriately received, stored and administered to patients;
  • Assess capacity, gain valid informed consent and work within the legal framework with patients who lack capacity to provide formal consent;
  • Extended clinical assessment (listening to vital signs) and interventions (venepuncture, cannulation and prescribing medication); 
  • To participate in multi-disciplinary activities such as case conferences, joint assessments, goal setting, team meetings and discharge planning;
  • To plan, implement and evaluate evidence-based programmes of care;
  • To work and maintain intermediate care nursing standards as agreed by my client and take responsibility to inform managers and leads when quality standards cannot be met; 
  • To participate in the pre-assessment of new patients as set out by the service objectives for Intermediate Care;
  • To provide clinical leadership, advice and expertise to staff within the service, as required.

 

The post holder will supervise, co-ordinate and deploy members of the nursing and home care team according to the requirements of the patients and where appropriate, to oversee other patients. You will support, guide, teach and mentor others in line with the regulatory frameworks, company training programmes and the NMC Code.  The post holder will act as an ambassador for the company during external visits to assess new residents and you will ensure compliance with all legal, regulatory and best practice guidelines – ensure risks are proactively managed and issues identified and reported appropriately.

 

The successful candidate will have:

 

  • NMC registration; 
  • Up-to-date knowledge of agendas around Intermediate Care;
  • Recent experience of delivering nursing care in a clinical environment;
  • Understanding of and competence in nursing procedures;
  • Awareness and understanding of diversity;
  • Understanding and knowledge of Safeguarding Regulations;
  • Understanding and application of relevant legislation in relation to the role:
  • Awareness and understanding of the CI/CQC/RQIA National Care Standards;
  • Previous experience working in an acute environment in either ICU, Medicine, Surgery or Care of the Elderly.

My client offers a salary between £45,000 - £50,000 per annum with a company car, pension and a benefits package. This role is suited to an ambitious professional, experienced at working at Band 7 level, we are interested to hear from individuals working within the NHS or Private Healthcare.
